Violation descriptions and comments

An employee was observed rinsing off the lettuce slicer in the food preparation sink, then placing it with other cleaned and sanitized equipment. Without being correctly washed, rinsed, then sanitized before storing the piece of equipment.
The door seal for the large ice machine was not properly attached to the door. A knife was found stored in the utensil bin that had a melted handle that was smooth and easily cleanable. In the vent hood over the fire griller had gaps in between the filters. The caulking on the 3 compartment sink is missing on the right hand sink of the sink. The caulking on the hand sink in the men’s bathroom is separating from the sink and wall.
The shelving that is used for storing of the food equipment was found soiled. The top of the hot holding units on the preparation lines were found soiled.
The food equipment was found to be stacked wet on the equipment storage shelving near the 3 compartment sink. The bucket of sugar was found to have the scoop in the sugar with the handle in the sugar.
The single service items that were stored on the top of the hot holding equipment were stored with the food contact surface facing up.
The wall behind the 3 compartment sink was found to have small nail holes in it. There is a hole in the wall adjacent to the cashier area. The tiles by the back door are chipping, cracked, or not attached to the sub flooring, as well as the coving tile next to the mop sink was found to be cracked.
A light bulb was found not working in the walk in freezer.
