Violation descriptions and comments

A Y valve with shut offs was attached to the mopsink faucet, thus allowing the integral atmospheric vacuum breaker to be left on continual pressure.
Numerous flies were present in the food preparation area.
A fly strip with no catch tray was present above the salad preparation table.
The interior of the double-door reach-in fry freezer had excessive ice buildup on the walls.
The front of the McFlurry ice cream dispenser was soiled with ice cream splatter.
Wiping cloths were stored in chlorine sanitizer solution with a concentration less than 50 ppm, in the fry line wiping cloth bucket.
A scoop with no handle was present in the drive through area drink ice bin.
Debris was present on the floor in the walk-in freezer and walk-in refrigerator. The floor drain below the three-compartment sink was soiled with debris and grime. The floor was soiled with greasy debris below the flat grill.
One light bulb did not function in the walk-in freezer.
One ventilation filter screen was missing in the french fry ventilation hood.
