Remarks: A/C - FORMER MILITARY MCDONNELL DOUGLAS A-4-N SKYHAWK. A/C WAS DESTROYED WHEN IT IMPACTED THE GREAT SALT LAKE FOLLOWING A LOSS OF ENGINE POWER AFTER INGESTING A BIRD NEAR SALT LAKE CITY, UT. THE SOLE OCCUPANT ON THE A/C WAS FATALLY INJURED. PILOT EJECTED AT APPROX 1,000 TO 1500 FEET ABOVE THE LAKE. THE EJECTION SEAT HIT THE TAIL OF THE A/C AND THE PILOT DID NOT SEPARATE. NO PARACHUTE DEPLOYED.
