An employee twist top beverage was stored on top of the dessert cooler.
Several live roaches were seen under the dessert cooler through a hole in the floor wall junction.

An employee twist-top beverage was observed on top of the dessert cooler.

An employee was observed handling dirty dishes and then clean dishes without first washing their hands.
Plastic spoons and a rag were observed in the basin of the front kitchen hand sink. Employees were observed wiping food surfaces with a common towel that had negligible concentration, less than 50ppm, of chlorine. The server station hand sink was being used for purposes other than hand washing as evidenced by ice in the hand sink basin.
Shredded chicken was 84�°F and ground beef was 80�°F after cooling for 3.5 hours in large pans in the walk-in cooler.
Mexican eggrolls were 111�°F on a plate on the bottom shelf in the open oven above the grill.
In the cold-top refrigerator, shedded cheese was 46�°F, cut tomatoes were 47�°F and cooked white fish was 45�°F.
Cold water was not supplied to the three-compartment sink. Hot water was not supplied to the bar hand sink.

Employees were using common towels with negligible concentration of chlorine to wipe hands and food contact surfaces on the cook-line.
Rice sitting out in pans was 10�°F after cooling for 2 hours.
