- Equipment and/or components are not maintained in good working order. The exhaust hood over the fryers is not sufficiently removing grease-laden air from the environment.
Repair and clean immediately to prevent buildup of grease on equipment, and fire hazards. Note: Ensure the entire stack is clean in addition to the filters.
- Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. Ice scoop is stored directly on top of the ice machine (not a cleand & sanitized surface).
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule. Wash, rinse, and sanitize the ice scoop immediately, and store properly in a clean and sanitized labeled container.

- Critical: Unsafe food was not discarded or properly reconditioned. A box of potato wedges stored directly under the condensor in the walk-in freezer was engulfed/contaminated in condensate water-ice. Condensate is a waste-water.
A food that is unsafe, adulterated, or not honestly presented as specified under 3717-1-03 of this code shall be discarded or reconditioned according to an approved procedure. Voluntarily discarded at time of inspection. Correct By: 14-May-2014
- Observed a build-up of dirt and debris. Dirty water covers floor surrounding 3-compartment sink. Roast was thawing under cold-running water, which was running "full-blast," and splashing onto the floor
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean floor, and ensure water is turned down to a slow flow for thawing so it doesn't splash onto the floor.
