- Observed improper method for cooling TCS foods. Observed a large pot (3 gallons) of beef tenderloin in sauce in the walk-in cooler (prepared foods).
To prevent the growth of pathogens: (1) Cooling shall be accomplished in accordance with the time and temperature criteria by using one or more of the following methods based on the type of food being cooled: (a) Placing the food in shallow pans

- Critical: Equipment food-contact surfaces or utensils are dirty. The ice machine interior of the Seafood Dept. was dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean and sanitize the interior of the ice machine as discussed.
- Observed floors, walls, and/or ceilings that were not constructed of approved materials. Observed bare wood 2x4 pieces being used as table legs and as shims in the seafood area.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable
- Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Back man door and overhead door are not adequately protected from rodent entry.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
- The physical facilities are not being maintained in good repair. The wall near the back doors is damaged.

- TCS foods were not properly thawed. *Observed fish in a tub of standing water for thawing.
TCS food shall be thawed as required in this rule. *Discontinue thawing items in standing water. Items may be thawed under cold running water or under refrigeration.
- Observed light bulbs or heat lamp that were not properly shielded or coated where required.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed. *Provide a cover/shield for the light bulb in the walk-in freezer in the prepared food area.
- Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. *Gaps beneath rear doors may provide access to pests.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule. *Eliminate any and all gaps to the exterior to prevent entry of pests.
- Critical: Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s). *No soap was observed in the employee rest-room (men's rest-room).
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. *Provide a constant supply of hand cleaner at all hand sinks where missing.
- The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. **Observed inadequate lighting in the walk-in freezer (prepared foods).
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning. *Provide adequate lighting in the walk-in freezer in the prepared foods area.
