Nonfood-contact surfaces of equipment shall be kept clean. Thoroughly clean these shelves and remove newspaper and wall coverings to facilitate their cleaning and deter pests.
- The physical facilities are not being maintained in good repair. The ceiling tiles are water stained or missing near the back room.
The physical facilities shall be maintained in good repair. Repair the roof leak/ceiling leak and replace bulging or missing ceiling tiles.
- Observed a build-up of dirt and debris. The toilet and the light switch in the restroom contain significant build up which could attract pests.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Thoroughly clean the toilet and the wall/light switch to eliminate build up and deter pests.
- Observed the presence of live insects, rodents, and other pests. Numerous gnats were seen flying around the freezer in the back room and the produce scraps on the floor in this back room.

- Critical: Observed food that was unsafe, adulterated, or not honestly presented. Two 5 lb containers of Desi Natural low fat yogurt were expired (date of 6/14/15) and in the glass front True dairy cooler.
To prevent foodborne illness, food shall be safe, unadulterated, and honestly presented. Discontinue selling out of date products to ensure their food safety. Corrected-employee moved the product to a cooler in the back and will voluntarily discard them.
- A label on a food packaged in the RFE or FSO did not contain all required information. Bulk grain items on the flour shelf contain only a "B" or "G" as their markings. These products do not contain the proper labeling.
Foods shall be labeled as specified in this rule. Bulk foods in sealed packaging must contain the common name of the product, the total weight or volume of the product, the ingredients in descending order from greatest to least, and the manufacturer name with city and state. Until these products can be properly labeled they must be removed from the sales floor-corrected during inspection.

Foods shall be labeled as specified in this rule. *All food items must be labeled with contents and source of item. Packaged meat was not labeled with contents, source, or date of packaging. Items were not in sales area of store and will not be offered for sale until properly labeled.
- Critical: Observed no towels or hand drying device at the handwashing sink(s).
Each handwashing sink or group of adjacent handwashing sinks shall provide hand drying by a means described in this rule. *Provide single-use towels in restroom for drying hands. Correct By: 21-May-2014
