- Critical: Ready-to-eat, TCS food that had been date marked was not properly discarded when required. Observed several containers of vanilla yogurt that had expired in 11/15.
To prevent illness, refrigerated, ready-to-eat TCS food that has been date marked in the facility shall be discarded if it: (a) exceeds the temperature or time specified in (G)(1) and (G)(2) of this rule, except the time the food is frozen
- Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s).
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. Operator provided soap at hand sink at end of inpsection.

- Observed improper storage of food items. Raw ground beef wa.s stored above ready to eat foods in the glass front kitchen cooler
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Corrected-operator moved raw meat below the ready to eat foods.
- Equipment is not approved by a recognized testing agency. Numerous homestyle equipment was noted throughout the facility including Kenmore, Goldstar and Oster microwaves, the stove/oven (beige) and the white homestyle refrigerator freezer in the infant room.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. When these units fail they must be replaced with NSF approved commercial grade equipment or the equivalent.
- Floors, walls, and/or ceilings were not smooth and easily cleanable. Carpet and flooring in the infant room and near the front entrance is worn and no longer durable.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Repair or replace this carpet within one year.
