- Critical: Food prepared in a private home was being used or offered for human consumption.
To prevent foodborne illness, except for products obtained from a home bakery registered by the Ohio Department of Agriculture or products as specified in 3717.021 of the Ohio Revised Code, food prepared in a private home may not be used or offered for human consumption in a food service operation or retail food establishment. [Do not prepare foods at home. Provide proper equipment for food preparation.]
- Critical: Observed improper reheating of food for hot holding.
To prevent the growth of pathogens, time and temperature controlled for safety (TCS) food that is cooked, cooled and reheated for hot holding shall be reheated as required by this rule. [Reheat frozen commercially prepared foods, like meatballs, rapidly, within two hours.]
- Critical: Observed inadequate equipment for cooling, heating or holding food.
To prevent the growth of pathogens, equipment for cooling, heating, or holding cold and hot TCS food shall be sufficient in number and capacity to provide food at the required food temperatures. [Provide proper equipment to prepare foods. All equipment must be National Sanitation Foundation (NSF) approved or equivalent.]
- Critical: There is no test kit available for measuring the concentration of the sanitizer.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided. [If bleach is to be used as a sanitizer, provide chlorine test papers.]
- Critical: Food-contact surfaces and utensils were not sanitized using an approved combination of exposure time, chemical concentration, temperature, and/or pH.
To prevent pathogen growth, after being cleaned, food-contact surfaces and utensils shall be properly sanitized by a method approved in this rule. [Only use chlorine bleach that does not contain a fragrance for cleaning and sanitizing utensils and surfaces.]
- The plumbing system was not properly maintained.
A plumbing system shall be properly maintained. [Repair or replace the hot water tank for the food service. Water heater broke over the weekend. Hot water can be obtained from other parts of the building to wash, rinse, and sanitize.]
- Critical: Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s).
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
- Critical: Observed the presence of live insects, rodents, and other pests.
