The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
- Observed employees cleaning a mop in a warewashing sink.
A warewashing sink may not be used for cleaning mops and or disposal of waste water as specified under paragraph (D) of rule 3717-1-02.2 of this code.
- Observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

The physical facilities shall be maintained in good repair. Maintain floors, walls, and ceiling in good repair. Repair holes in walls, chipping and flaking paint, missing and damaged ceiling tiles etc. All sufaces must be smooth and easily cleanable.
