Equipment and/or components are not maintained in good working order (ie. hot holding display case in the deli department is not maintaining an ambient air temperature of at least 135 degrees Fahrenheit, therefore, stored foods are not being hot held at or above 135F). Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair hot holding display case in the deli department (ie. used for broasted chicken, pizza, potatoe wedges, etc.) in order to ensure cooked items are being held hot at 135F or above until purchased.

Eliminate build-up of ice/frost on ceiling and floor of the deli meat walk-in freezer to eliminate slip hazard and potential contamination of stored food items. NOTE: Should condensation drip onto stored food products and they become encased in ice/frost, the product must be discarded. Condensation is considered sewage.
Observed a build-up of dirt and debris and standing water underneath and behind the large mixer in bakery dept (ie adjacent to 3 compartment sink). The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Eliminate standing water and clean underneath and behind equipment near the 3 compartment sink in the bakery dept. Maintain clean and dry on a daily basis.

Standard Inspection
A label on a food packaged in the RFE did not contain all required information (ie. ingredient labels are missing for the buffalo chicken dip and the spinach dip made in house). Foods shall be labeled as specified in this rule. Provide ingredient labels as discussed for the buffalo chicken and spinach dips that are made in house, packaged and placed on the retail floor near deli area.
Equipment and/or components are not maintained in good working order (ie. ice continues to be present on the ceiling and floor of the ice cream walk-in freezer below and adjacent to condensor). Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Remove ice from ceiling and floor of the ice cream walk-in cooler to eliminate potential product contamination and employee slip hazard.

Equipment and/or components are not maintained in good working order. Observations in the following areas were: 1. Missing electrical outlet plate on wall of produce prep area. 2. Handle of chilli scoop has been covered with tape which is now falling off. 3. Ice on floor of ice cream walk-in freezer. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Address the following areas: 1. Replace missing electrical outlet plate in produce prep area. 2. Remove tape from handle of chilli scoop so it can be properly cleaned and sanitized. 3. Remove ice from floor of ice cream walk-in freezer to eliminate slip hazard.
Non-food contact surfaces of equipment are dirty. The following observations were made: 1. Fan shrouds in the produce and dairy walk-in coolers are covered in dust. 2. Water spray lines above the produce contain mold. Nonfood-contact surfaces of equipment shall be kept clean. Address the following: 1. Clean fan shrouds inside of walk-in coolers where needed to remove dust/dirt. 2. Clean water spray lines above the produce to remove mold growth. NOTE: Place items on a regular maintenance log to ensure regular cleaning.

Standard Inspection
Equipment and/or components are not maintained in good working order. Observed was a build-up of frost/ice in the main walk-in freezer from condensor area and water dripping into a bucket on the floor from a condensor in the meat dept. butchering area. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Service the main walk-in freezer to eliminate dripping water and large build-up of ice/frost. Service condensor in the meat dept butchering area to eliminate dripping water.
The physical facilities are not being maintained in good repair. 1. Observed damaged walls on the rear wall of the meat dept butchering area (bead board) and on the side wall in the deli prep area (near 3 comp sink). 2. GFI electrical plug and plate are broken on the side wall of the meat dept butchering area (above wrapping machine). The physical facilities shall be maintained in good repair. 1. Restore the above listed walls back to a smooth, durable, easy-to-clean state (remove old mold-laden wood paneling above 3 comp sink in the meat deli area). 2. Replace broken electrical plug and plate on the wall (above wrapping machine) in meat dept.
