To prevent the growth of pathogens, equipment for cooling, heating, or holding cold and hot TCS food shall be sufficient in number and capacity to provide food at the required food temperatures.
- Observed floors, walls, and/or ceilings that were not constructed of approved materials. Replace acoustic ceiling tiles in kitchen with smooth and cleanable tiles.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable
- Observed no towels or hand drying device at the handwashing sink(s). Provide constant supply of hand towels.
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels
- Observed a build-up of dirt and debris. Clean floors and sides of equipment to remove buildup.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

- Critical: The person in charge was unable to demonstrate proper knowledge of food safety and prevention.
To prevent or correct factors that may cause foodborne illess, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection. Kitchen staff did not demonstrate a working knowledge of the minimum cook temps for the various raw meats utilized at facility nor did they seem to be aware of the time/temp parameteres for bulk reheats of PHF. The temps and temp/time parameters are as follows: Minimum cook temps: pork/seafood - 145F
- Critical: Observed food that was not properly protected from contamination by separation, packaging, and segregation.
Plastic container of garnishes found nesting directly in ice bin with ice that was to be used for drinks. Ice is considered food and as such no outside containers are to be placed directly onto food's surface. - Nonfood-contact surfaces are not designed and constructed to allow easy cleaning or maintenance.
Nonfood-contact surfaces shall be designed and constructed to allow easy cleaning and to facilitate maintenance. Eliminate shelving units in dry storage area that have wood/press board shelf leaflets. Press board can not be properly maintained with regards to sanitation i.e., it is an absorbant material that will hold moisture and be prone to mold growth.
- Equipment components are not intact, tight or properly adjusted.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Replace damaged gasket liner for door for upright refrigeration unit so unit can run more efficiently and sanitation can be maintained.
- Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean. Thoroughly clean the interior of the beer keg coolers to eliminate all standing water/accumulate as discussed.
- Critical: Observed the air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water.
Provide 2" air gap for drain lines for all ice bins at bar and ice machine drain line to eliminate the potential for the back siphonage of sewage into ice stocks in case a sanitary sewer back up did occur. Correct By: 23-Jan-2015

- Observed an outer opening, window, or door without proper protection against the entry of insects and rodents.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule. Replace exterior doors ( at locale where ice machine is located) to facility where large gaps are present at bottom of doors and inbetween doors. Plastic garbage bags are currently in place to keep out cold. Likely this issue was the means for the access of unwanted pests/mice into the facility. The adjacent exterior entrance at the back of the facility does have one small gap which could be corrected w/ a new door sweep/guard.
- Observed a build-up of dirt and debris.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Thoroughly clean floors under equipment on cook line to address dirt/accumulate.
- Critical: Observed the presence of live insects, rodents, and other pests.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies
- Observed the presence of dead pests or insects.
Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. Remove dead mice from glue trap behind pulbing access panel for reasons cited above.
