- Observed inadequate number of handwashing sinks. There is no hand sink in the rear of the store for use by employees that handle foods.
To ensure frequent and proper handwashing, at least one handwashing sink, a number of handwashing sinks necessary for their convenient use by employees in areas specified under paragraph (L) of this rule, and not fewer than the number of handwashing sinks required by the Ohio building code shall be provided. Regional manager and store staff indicated that a work order has been written for a sink.
- Observed a build-up of dirt and debris. Rear room is still unorganized and unclean.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Suggest reorganizing this area in a manner that facilitates the ease of cleaning floors and walls.

- Non-food contact surfaces of equipment are dirty. The interior of the walk in cooler is dirty.
Nonfood-contact surfaces of equipment shall be kept clean. Clean the floor of the walk in regularly to remove spilled food and debris.
- Observed inadequate number of handwashing sinks. No handwashing sink exists outside of the public restroom.
To ensure frequent and proper handwashing, at least one handwashing sink, a number of handwashing sinks necessary for their convenient use by employees in areas specified under paragraph (L) of this rule, and not fewer than the number of handwashing sinks required by the Ohio building code shall be provided. Provide a hand sink in the back room for use by employess for the prevention of food contamination.
- Observed a build-up of dirt and debris. The back room floors and walls are dirty.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Operator must clean this area to remove all buildup from the floors and walls and remove any unneeded items from the area.
