Observed improper use and/or maintenance of wiping cloths. Observed wiping cloths sitting around prep area. These need to be placed in a bucket of sanitzer solution or accompanied with a bottle of sanitizer solution so that prep areas are being sanitized when cleaning (1) Cloths that are in use for wiping food spills from tableware and carry-out container that occur as food is being served shall be maintained dry and used for no other purpose
TCS foods were not properly thawed. Observed inappropriate thawing of prepackaged streak. PIC left it to thaw on prep table TCS food shall be thawed as required in this rule.Thawing is appropriate by placing the food items under cold, running water or cooking the food item while still frozen or thawing in the cooler or by microwaving and then immediately cooking.
Critical: TCS foods were not being held at the proper temperature. To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.
Equipment and/or components are not maintained in good working order. Measured prep tables above 41 degrees F. (Procedure in place to adjust prep tables and retest food temperatures) Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. All cooling units must be adjusted below 41 degrees F to properly hold cold foods at appropriate temperatures

Probe thermometer is not calibrated to appropriately measure temperatures for food Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Thermometers must be calibrated frequently enough to give accurate readings.
Critical: Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material. To prevent health hazards, working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.

Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Repair/replace baackdoor to seal properly. To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
