  • Critical: Food employee(s) are not using proper procedure when washing hands and arms. Unable to wash hands in warm water above 100 degrees F
    To prevent contamination, food employees shall clean their hands and exposed portions of their arms including surrogate prosthetic devices for hands or arms in a handwashing sink as specified in 3717-1-02.2 (B).
  • Critical: TCS foods were not being held at the proper temperature.
    To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.
  • Critical: A quaternary ammonium sanitizing solution did not meet the minimum requirements for temperature, concentration, and/or water hardness.
    To prevent pathogen growth, a quaternary ammonium sanitizing solution shall: (a) Have a minimum temperature of 75F,(b) Have a concentration as specified and as indicated by the manufacturer's use directions included in the labeling, (c) Be used only in water with 500 ppm (mg/l) hardness or less, or used in water having a hardness no greater than specified by the manufacturer's label.
  • Observed a handwashing sink without water at the required temperature.
    To ensure proper cleaning of hands, a handwashing sink shall be equipped to provide water at a temperature of at least 100° F (thirty-eight degrees Celsius) through a mixing valve or combination faucet.

    Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Corrected - temperature setting adjusted.

Wall paper in back of kitchen (next to walk-in cooler and freezer) is tearing. A large crack/bump can be observed in the floor in front of the back door.     The physical facilities shall be maintained in good repair. Repair coving and wall to provide a sanitary and easily cleanable surface. Repair crack in floor tiles to provide an easily cleanable surface.
  • The physical facilities are not being maintained in good repair. Observed mold behind 3 comp sink, where sink is sealed to the wall. Sealing was begin to tear from wall along right side of sink.
    The physical facilities shall be maintained in good repair. Replace sealing to provide a sanitary surface and prevent contamination of food contact surfaces.

  • Observed mops being dried using an unapproved method. Observed mop being stored with head on the floor.
    After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. Hang mop to allow for proper air drying.
