  • Observed food employee not washing hands properly. The hand sink was not stocked, had items stored in it, and was dry.
    Food employees shall wash their hands as specified under paragraph (B) of rule 3717-1-02.2 of OAC. Employees must wash their hands as often as required, especially after handling raw chicken and fish.
  • Critical: TCS foods were not being held at the proper temperature. Fried fish, chicken, cheese sticks, etc. were found at 107 F - 118 F in the display case.
    To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less. All food products were reheated by the operator to at least 165 F.
  • Observed an improperly cleaned storage area for refuse, recyclables, or returnables. There is a heavy accumulation of grease on the dumpsters.
    A storage area and enclosure for refuse, recyclables, or returnables shall be maintained clean and free of unnecessary items.
  • Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s).
    To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
  • Observed no towels or hand drying device at the handwashing sink(s).
    To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels

  • Critical: Food employee(s) did not wash hands in situations that specifically require them to do so. Observed employee go from handling money at the cash register to gathering food. Put on gloves but did not wash hands prior
    To prevent contamination, food employees shall clean their hands and exposed portions of their arms after switching between working with raw food and with ready to eat food
  • Food prepared in a private home was being used or offered for human consumption. Miss Mays Home Goods sold at this facility without proper labeling. Need to determine if this food is TCS. i/e: Cream cheese frostings for cakes
    To prevent foodborne illness, except for products obtained from a home bakery registered by the Ohio Department of Agriculture or products as specified in 3717.021 of the Ohio Revised Code, food prepared in a private home may not be used or offered for human consumption in a food service operation or retail food establishment.
  • Observed improper use and/or maintenance of wiping cloths.
    (1) Cloths that are in use for wiping food spills from tableware and carry-out container that occur as food is being served shall be maintained dry and used for no other purpose
  • Miscellaneous sources of contamination observed. Observed containers of raw chicken left uncovered in a public reach in cooler
    Food shall be protected from contamination that is not specified under 3717-1-03.2 (A) to (X).
  • Critical: Unsafe food was not discarded or properly reconditioned. Observed 2 food items that were semi open and damaged in the reach in cooler
    To prevent the transmission of foodborne illness, a food that is unsafe, adulterated, or not honestly presented as specified under 3717-1-03 of this code shall be discarded or reconditioned according to an approved procedure.
  • Critical: The concentration and/or temperature of a chlorine sanitizing solution did not meet the minimum requirements specified in the code. Bucket with sanitizer also was soiled.
    To prevent pathogen growth, a chlorine solution shall have a minimum temperature based on the concentration and pH of the solution as specified in the code.
  • Floors, walls, and/or ceilings were not smooth and easily cleanable.Observed worn walls, bulging ceiling tiles, and missing ceiling tiles
    Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
  • Observed a build-up of dirt and debris. A general cleaning is needed throughout facility i/e: kitchen area, walk in cooler, reach in cooler spillage, and dry storage area where major litter was observed. Also shelving needs cleaned throughout store
    The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
  • Observed mops being dried using an unapproved method. Observed mop in utility room stored in bucket of soiled water
    After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
  • Critical: Observed the presence of live insects, rodents, and other pests.
    To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies
  • Observed unnecessary or nonfunctional items and /or litter on the premises. Observed excessive trash and debris at dumpster area
    The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

  • Critical: Observed food that was unsafe, adulterated, or not honestly presented. **Expired baby formula was observed/offered for sale.
    To prevent foodborne illness, food shall be safe, unadulterated, and honestly presented. **Voluntarily remove and discontinue offering for sale, outdated food items (baby formula). Corrected.
  • TCS foods were not properly thawed. **Chicken was observed thawing in the sink without running water.
    TCS food shall be thawed as required in this rule. **Thaw items under refrigeration or under cold running water as discussed. Chicken was moved into a cooler to complete thawing.
  • A food packaged in the facility did not contain a label. **Cheesecake observed in the display cooler without labels.
    Foods shall be labeled as specified in this rule. **All items must be labeled with ingredients, manufacturer, etc. as discussed.
  • Critical: Observed no towels or hand drying device at the handwashing sink(s).
    To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels
  • Observed unnecessary or nonfunctional items and /or litter on the premises.
    The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. **Remove all unnecessary items in facility to allow for easy cleaning throughout the store.
