- Observed equipment in poor repair. Lock the pad lock, in the open position, every time that the walk-in-refrigeration units are open, so that no one can be locked inside.
- Nonfood-contact equipment not designed and constructed in a durable manner. Remove the ice dam from the walk-in freezer-door jamb.
- Observed walk-in cooler gasket torn/in disrepair. Repair the walk-in freezer-door gasket.
- Critical. Vacuum breaker mising at hose bibb. Install a vacuum breaker, on each hose connection, at the three-compartment sink.
- Critical. Observed spray hose at dish sink lower than flood rim of sink. Secure the dishspray nozzle, at the three-compartment sink, so that the distal end remains above the sink rim at all times.
- Critical. No proof of required employee training provided. All public food service establishments must provide the division with proof of employee training upon request, including, but not limited to, at the time of any division inspection of the establishment. Employeess must pass Food Safety Training within 60 days of hiring. Keep proof on the premises.
