Observation:Light shields in the food prep area did not have end caps.

Observation:Facility needs an employee who is a certified food protection manager.

Observation:Employee health forms were not present for review.

Observation:Food was being stored on the floor in the walk-in cooler. (Lettuce)
- Physical facilities maintained in good repair
Observation:Back dining area needs to be thoroughly cleaned and the ceiling needs to be repaired. Excess clutter needs to be removed.
- Material characteristics of non-food contact surfaces
Observation:Walk-in cooler needs a ceiling with a surface that is smooth and easily cleanable. Considerable mold buildup currently on the ceiling.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:Thermometers were not present in all cooling units.

Old dressings in the cooler need to be thrown out. Corrected at time of inspection

Area around the fryers and grills needs to be cleaned. Ceiling area of the walk-in cooler needs to be a smooth and easily cleanable surface.
