Observation: No verifiable Employee Reporting Agreement available in establishment. Inspector gave person in charge a copy of the FDA form 1B reporting agreement to review and have staff sign.

- Sanitization methods - hot water, chemical
Observation: Employee washing blender mixers at the three compartment sink was not sanitizing the mixers after washing. The mixers were only being washed and rinsed. PIC set up the three compartment sink to wash, rinse, and sanitize with chlorine solution. All mixers were then rewashed correctly.
- Test kit provided and used to measure sanitizing solution concentration
Observation: Establishment is out of chlorine test strips to check the sanitizer solution.

Observation: Employees tea drink with no tight fitting lid stored on the counter by the register above single service items. Drink discarded by PIC
- Using a handwashing sink- operation and maintenance
Observation: A table with blender mixer containers air drying was placed in front of the prep line area hand sink blocking access. PIC moved the table to allow access to the hand sink.
