Observation: Cardboard was observed being reused to hold items. Cardboard may only be used for the original product and then it must be discarded.

Observation: Pepsi and Gatorade Retail cooler doors have torn door gaskets. Also the white plastic tub by the kitchen walk in freezer is broken. Tub must be replaced or removed.

Observation: Found a dented can of vienna sausage in the retail area. May not sell any can that is dented on top, bottom, or side seals. Also any can that has a gap when set on level surface or has sharp points due to denting. Can was discarded at time of inspection.

Observation: Pop nozzles and cappuccino dispenser have become soiled and must be cleaned.

Observation: Sandwich cutting boards are stained/scored and must be resurfaced or replaced.

Observation: Prep sink by the pizza oven is not in operation due to splashing from the drain line. Sink must be properly abandoned or maintained in good working order.

Observation: The following areas have become soiled and must be cleaned: legs of floor equipment in kitchen, behind floor equipment in kitchen, pizza prep cooler (inside), and lid/cup dispensers in retail.

Observation: Staff member using soap to clean food contact surfaces (counter tops in kitchen area. Must use food grade sanitizer to properly clean food contact surfaces. Corrected on site with education.

Observation: Bolt missing on right side of pizza reach-in preparation cooler top lid. Repair to ensure top lid door closes properly and is maintained in good repair.

Observation: Food build-up, spilled stained, and some mouse droppings found in several cupboards throughout facility. Has monthly pest control for rodent and insect issues. MUST CLEAN FREQUENTLY TO PREVENT ENVIRONMENT FOR PESTS.

Observation: 1) Food build-up on lid for doughnut flour bin. 2)Food and grease build-up inside shelving and exterior doors of reach-in pizza preparation cooler. 3) Clean inside hot chocolate machine 4) Mold growth on front of ice chute inside large ice machine. Facility cleans machine every month. Ensure to wipe inside front of ice cute with sanitizer to clean of heavy condensation build-up and help prevent growth. Must clean all food contact surfaces as often as necessary to prevent food build-up and contamination .

Observation: Food preparation sink next to pizza preparation cooler is currently not being used due to leaking problems. Kitchen has another food preparation sink across the room. Must repair sink to maintain in good working condition. REPEAT VIOLATION.
