Observation:The license will need to be posted consistent with the inspection report.

Observation:The inspection report is currently posted > 7 ft up and back on a wall that customers should not be in. This will need to be moved to another area where customers can read it approximately eye level.

Observation:opened bags of cheese, ham etc...missing datemarks
- Food storage - preventing contamination from the premises
Observation:paper sack of dough dusting powder is sitting on floor.
- Miscellaneous sources of contamination
Observation: cutting board on floor (x2) food containers on floor, cotton candy barrel is on the floor in storage, lids for glasses are sitting on the floor, garbage can is holding food containers nearest to the salad cooler. Please wash-rinse-sanitize and air dry these food contact surfaces(FCS) in a sanitary manner. FCS need to be at least 6" up off of the floor and protected from contamination.

- Food temperature measuring devices are accurate to ±1°C or ±2°F in the intended range of use
Observation: bi-metallica stem thermometers are all showing a different temperature in excess of 10 degrees both ways

- Roasts held at a temperature of 130°F or above
Right side (closest to pizza oven) of pizza prep cooler- some temps are at 45+. Monitor these and ensure a cold hold of 41 or less. in the lower area- air temp is around 39F. Consider using metal food grade safe pans to help facilitate proper cold hold.
- Reportable symptoms, diagnosis, past illness, and history of exposure.
Employee health reporting agreement is needed for all staff. FDA Employee Health and Personal Hygiene provided. Form 1-B is needed or facility generated documents that include signs and symptoms, the Big 5 and household exposures.
- Bare hand contact with ready to eat foods
Observed staff handling ready to eat foods (lettuce) with bare hands. Food service gloves or tongs may be used. Please review requirements as it pertains to ready to eat foods.
