    Observation: Utility sink used as a service sink was cracked/worn and water was leaking into a bucket under the sink. Person in charge stated they will purchase a new service sink. Ceiling of storage room where deep chest freezers are stored had insulation that had black residue build-up and was covered with plastic tarp
  • Maintaining premises free of litter and unnecessary equipment
    Observation: Establishment had unnecessary items stored by the front exterior door behind the counter, such as a broken ice machine. Person in charge has begun clearing items out of front area and there is a path cleared to walk behind the counter top. Establishment has an unused safe in the kitchen, next to door leading to the back room. Unnecessary items stored on shelves and corners of the back storage room where the deep chest freezers and walk-in-cooler is located. Person in charge has begun clearing more items.

    Observation: Light bulbs were burned out in the kitchen. Establishment had a lamp available inside.
  • Multiuse food contact surfaces are constructed of materials that are: durable, corrosion-resistant, non absorbent
    can withstand repeated warewashing

    Observation: Light bulb tubes hung are not shatter resistant and do not have a protective shielding in the kitchen. Person in charge has purchased light bulb tube covers and stated they will cover bulbs.

    Observation: Fan in kitchen was soiled. Person in charge stated that they would remove and discard fan.

    Observation: Ceiling of storage room where deep chest freezers are stored had insulation that had black residue build-up and was covered with plastic/tarp, was leaking onto a bowl stored on chest freezer. Floor of walk-in-cooler was worn and broken. Person in charge stated they are not using the walk-in-cooler. Broken tiles in the kitchen and some behind the bar. Holes in walls of back storage room showed exposed insulation. There was a broken wall next to storage room door leading into main kitchen. Ramp leading from kitchen to storage room unsealed. Door leading to storage room from kitchen is worn and in bad repair.

    Observation: Fan in kitchen was soiled. Wall behind standing freezer and refrigeration in kitchen had residue build-up.

    Observation: What appeared to be deceased larvae in the pop gun holster. Person in charge removed deceased insects.

    Observation: Exterior of small deep chest freezer had residue build-up. Shelving under microwave and next to safe had residue build-up. Deep chest freezer in back storage room had residue in interior of unit. Person in charge has removed ice build-up inside unit.

    Observation: Raw ground beef patties stored above pork loins. Person in charge rearranged refrigeration shelf in kitchen.

    Observation: Eggs stored above beverages in the reach-in refrigeration unit at the bar. Open door refrigeration contained raw pork and ground beef stored above food items in the kitchen.

    Observation: Interior of refrigeration soiled in kitchen and sliding door refrigeration containing eggs at bar. Fryer's exterior in kitchen had residue build-up. Yellow shelving unit across from three-compartment sink was soiled. Milk crates stored on the kitchen floor soiled. Large white buckets stored on floor of kitchen exterior soiled. Utensil storage containers at bar near three-compartnet sink was soiled. Exterior of equipment such as top of freezer had residue build-up. Interior of deep chest freezer had ice build-up.

Broken wall next to storage room door leading into main kitchen. Ramp leading from kitchen to storage room unsealed. Door leading to storage room from kitchen is worn in in bad repair.

    Observation: Peanut shells on floor next to bar by the bar stools. Bucket under ice bin at bar contained standing water. Floors soiled behind bar, kitchen, and storage room. Person in charge began cleaning floors behind bar during time of inspection. Walls soiled with food splatter throughout the kitchen and by fryers. Plumbing fixtures under bar three-coompartment sink soiled. Person in charge cleaned fixture during time of inspection. Behind equipment pipe fixtures were soiled and had grease residue build-up. Fan in kitchen soiled. Rubber floor mats behind bar and in kitchen were soiled.
