Observation:Observed baked potatoes cooled in stacked, tightly covered pans, and roast cooling in tightly covered pan.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:There is no thermometer in salad prep cooler.
- Cleaning procedure
Observation:Observed kitchen employee frequently use dry cloth towel to wipe hands, instead of washing hands at handwashing sink. Corrected on site.
- Bare hand contact with ready to eat foods
Observation:Observed employee butter and slice bread with bare hands
- Hand drying provided
Observation:There are no paper towels at bar handsink. Corrected on site.
- Cooked PHF/TCS food cooled from 135°F to 70°F within 2 hours and from 135°F to 41°F or below in 6 hours
Observation:Observed roast in tightly covered container on bottom shelf of walk-in cooler, temperature of roast was 48 degrees.
- Plumbing system maintained in good repair
Observation:The hot water handle at two-compartment sink was shut off - drips when in use - the water has to be turned on at the valve under the sink. Kitchen handsink has a slow drain. Observed container of water under water line to dishwashing machine - owner said there is a slow leak there.
- Certified Food Protection Manager with management and supervisory responsibility employed by the establishment
Observation:There is no certified food protection manager employed - kitchen manager said she has taken a ServSafe course but is not certain if the certificate is still valid.
- Handwashing signage
Observation:There is no handwashing sign for employee restroom. A sign was printed, corrected on site.

Observation:Observed missing light covers in fixtures in kitchen.
- Disclosure of menu items offered or served raw or undercooked
Observation:There is a consumer advisory in the menu, but items served undercooked (ground meats such as burgers and red meats) are not disclosed with an asterisk.
- Equipment location, installation, repair, and adjustment
Observation:Observed kitchen reach-in cooler is blowing hot air, needs new coil according to owner. Observed large ice buildup inside kitchen freezer, and another kitchen reach-in cooler has a bent fan blade and moisture is collecting on the doors. Ice machine is not in operation - filters are clogged and the bin has sediments that have collected in the bottom.
- Plumbing system maintained in good repair
Observation:The drain hoses are not yet connected to the bar ice bins and the fountain pop ice bin in the kitchen. The hot water faucet handle at the two-compartment sink is not in good working condition - hot water does not turn on.
- Cleanability of floors, walls, and ceilings
Observation:Observed missing floor tiles in beer keg walk-in cooler and the entrance to the cooler in the storeroom, and inside the kitchen walk-in cooler.
