- Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
Observation: Dust accumulation on the ceiling, walls, light fixtures, and ceiling vents. Grease and food debris accumulation on the floor underneath the cooking equipment.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: Piercing blade on the can opener is soiled. Can opener placed in the warewashing area to be cleaned and sanitized.

Observation: Storage container for flour is not labeled.

Observation: Non closed employee beverage sitting near the pizza prep table. Beverage was placed in a cup with a lid and moved to a location away from prep area.

Observation: Several cans found in storage with severe dents affecting the end seams. Cans were removed from storage and placed in an area designated for return to vendor.

Observation: Wet wiping cloths not stored in a sanitizer solution between uses.

Observation: Raw fresh shell eggs stored over ready to eat items in the display case. Raw hamburger patties and bacon stored over ready to eat items in the reach in cooler. Corrected on site.

Observation: Pasta salad made in the establishment dated 5/1/14 and 5/9/14 found in storage. Salads were discarded voluntarily.

Observation: Raw hamburger stored over bacon in the reach in cooler. Corrected on site.

Observation: Establishment does offer burgers and steak sandwiches cooked to order if requested. No consumer advisory is posted for such items.

Observation: Ice and other food debris dumped into the handwashing sink in the sandwich prep area.
- Single use glove use
Observation: Single use gloves used to place raw hamburger patties on the flat top grill. Same gloves were used to grab hamburger bun. Employee was instructed on proper single glove use at time of violation.
