Bare hand contact with ready to eat foods Observation:Observed several food handlers touch ready to eat foods with bare hands. This has been a violation at previous inspections. Staff put gloves on after inspector's arrival.
Material characteristics of non-food contact surfaces Observation: cloth towels used under glasses.
Light bulbs, protective shielding Observation:Lights are not properly shielded. over food prep, dishwash and in back storage.
Disclosure of menu items offered or served raw or undercooked Observation:Items on menu that may be served at less than well done need to be denoted by an asterisk. (consumer advisory is on menu) Hand correct or correct with next menu printing.
Food shall be obtained from sources that comply with law Observation:Multiple waterfowl found in freezer. These are not from an approved source. All foods in licensed facility are to be from an approved source.

Plumbing system maintained in good repair Observation:Hot water shut off at restroom handsink, water shut off at handwash sink near pop machine. Handwash sinks need to be in proper working order.
Single -service and single-use articles may not be reused Observation: Chocolate milk jugs with orange juice in them. Milk jugs are single service containers and can not be re-used. correct this ASAP.
Outer openings are protected Observation:mouse droppings noted in some areas. Tighten up outer openings, seal up wall around dishwash area, close off openings in ceilings. clean up droppings. (around ice machine, check under and around equipment)
Light bulbs, protective shielding Observation:Light shields are missing over cooking, warewashing and storage areas.
Handwashing signage Observation:Signs are missing for restrooms. "Employees must wash hands before returning to work." (like sign on back kitchen door)
Physical facilities maintained clean: Cleaning frequency, restrictions, and methods Observation:Walk in cooler has soiled areas, storage racks have build up on them. Deep cleaning needed in walk ins.
When to wash Observation:Observed staff return to back kitchen and they did not wash hands when they returned to work. Please review handwashing requirements.
