Teardown equipment/thorough cleaning Failure to take apart equipment as necessary to ensure thorough cleaning. Specifically, the Oliver brand bun slicing machine had food residue present in the upper feed/slicing chute. The slicer was last used for hamburger and hot dog bun production on 1/8/2014. Personnel stated that the machine had not been fully disassembled for cleaning. Labeling Food labeling and ingredient content declaration: Failure to declare all ingredients within a manufactured food. Specifically, Pita bread (Whole Wheat Pita Bread) and cut Hamburger and Hot Dog buns contain high gluten flour (bleached, enriched, and bromated).
