Observation:The ice chute on the pop machine has build up in/around it. Bases of pop nozzles and in between bases there is build up. area needs to be detail cleaned. Put this in a routine cleaning list. Ensure staff is cleaning thermometer between uses. (very soiled with various foods)

- Reportable symptoms, diagnosis, past illness, and history of exposure.
Staff does not remember signing employee health and reporting agreement. Please review form 1-A and 1-B of the booklet provided. Ensure all current staff review and sign 1-B. (reporting agreement must include the signs and symptoms listed, the Big 5 illnesses and include houshold exposures.
- Linens- cleaning and storage
Store utensils with handles out. (by cash register plastic knives-some have food contact side pointing upward.)
- Storage and maintenance of wet and dry wiping cloths
wiping cloths out on counters....keep in solution between uses
