    Observation: Pop spickets were not clean to sight. The units were cleaned prior to me leaving.
  • Certified Food Protection Manager with management and supervisory responsibility employed by the establishment
    Observation: The PIC stated the owner has the certification of serve safe but could not provide a certificate.
  • Storage of clean linens, single-service, and single use articles
    Observation: The dispenser unit for the single use containers did not have a top on it to protect the cups.
  • Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
    Observation: The vent hood above the fryer units was not clean to sight.
  • PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
    Observation: Shredded cheese was 45 degrees F. The cheese was in a bag and then placed in a pan in the fridge unit to cool to 41 degrees F.

    Observation: Bleach water tested out at 0 ppm. The unit was remixed to 50 ppm.

    Observation: The thermometer in the chest fridge was damaged and not accessible. The unit was replaced.

    Employees were not wearing hair restraints while prepping food.

    The facility has a sick policy but has failed to have everyone sign the policy. The person in charge could not remeber the five food borne illnesses and the five symptom of food borne illness.
