    Observation:Observed bag of mayonnaise that had a chewed hole in it, stored on walk-in cooler shelf next to a glue trap. Discarded, corrected on site.

    Observation:Observed rodent droppings on walk-in cooler shelf where the bag of mayo was located.

    Observation:Observed half a flat of shell eggs, container of hash browns and cheese slices stored in kitchen at room temperature.

    Observation:Observed openings in outer and inner walls of walk-in cooler, with visible signs of rodent droppings at each of the openings.

    Observation:Observed undated mashed potatoes, cooked hamburger, etc. in walk-in cooler.

    Observation:Observed raw roast stored on top of beer cans in walk-in cooler.

    Observation:Observed opening behind kitchen refrigerator that has several rodent droppings at the opening - the opening is between this establishment and the church building next door.

    Observation:Observed rodent droppings on walk-in cooler shelves and on top of food containers in walk-in cooler, on kitchen and storeroom shelves, and on floors under shelves and around equipment in kitchen. Discussed cleaning with hot water and soap, rinsing and then sanitizing surfaces with bleach water.

    Observation:Observed bags of sour cream and onion potato chips, hamburger buns, salt, pasta, and gravy mix with signs of entry from pests, and other foods in non-washable packages that were located on shelves with visible rodent droppings - voluntarily discarded and listed on a voluntary destruction certificate, corrected on site.

    Observation:Observed 3 sleeves of styrofoam soup bowls stored on shelf that has numerous rodent droppings - one sleeve had been chewed into and some bowls had visible chew marks. Discarded, corrected on site.

    Observation:Establishment opened for business under new ownership on July 1. The application was not processed until mid-July - establishment opened without a license and preopening inspection.
  • Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
    Observation:Observed undated corned beef, and containers of mashed potatoes and roast beef with date of 7/17 - foods were discarded, corrected on site.

    Observation:Observed potato fry cutter was heavily clogged with rotted potato pieces, taken apart, cleaned and sanitized, corrected on site.
