KDA_6_501111A - --One live cockroach was seen on top of north RIC in kitchen. COS: PIC disposed of cockroach. Facility has several glueboards throughout kitchen and storage area, all clear KDA_7_20212A2 - --Establishment has two different kinds of bleach used for sanitizing. One is scented and labeled "not for use as a sanitizer or disinfectant". COS: PIC designated scented bleach to be returned. Only non-scented bleach will be used for sanitizing food contact surfaces. KDA_7_20711B - --Employee's medicine was stored on shelf above containers of garlic and mustard. COS: Medicine moved to office. KDA_Defacto - --Employee was washing and rinsing dishes at three compartment sink, then stacking dishes to drain in third vat of sink. Person in charge (PIC) said dishes were being prewashed and dishes would be washed properly at a later time. --Frozen good. --North reach in cooler (RIC): American cheese - 42.9F. KDA_6_501111A - --On glue boards behind reach in coolers (RIC) in kitchen, there were 2 living and 4 dead baby cockroaches. When RICs were pulled away from wall, there was one dead adult cockroach in the back of the first RIC and four live adult cockroaches in the back of the second RIC. Person in charge (PIC) killed cockroaches and replaced glue boards with new. No issues were observed in any other area of establishment. PIC said he was going to contract a pest management company to assist until this issue is completely resolved. KDA_7_20212A2 - --PIC used a can (EPA Reg.
