  • KDA_3_30511A1 - --Located in the walk in freezer (WIF), there was a box of roasted potato medley, three boxes of turkey breasts, and a box of chocolate brownies stored on the floor.
  • KDA_3_50115A - --Located on prep table 2, there was a large container of gravy with a lids on it that was cooling after the chef had taken it off the stove. COS- educated on proper cooling methods (using smaller pans, or using an ice paddle), moved gravy into smaller containers. Located in the WIC 2, there were two large containers of turkey juice and turkey gravy with lids on them at 93F and 119F, respectively. The PIC stated that they had just put them into the WIC within the hour. COS- educated on proper cooling methods. Moved gravy into smaller portions and put in freezer with lids off.
  • KDA_3_50116A2 - --Located in WIC 1, there was a pan of alfredo pasta that was at 57F. The chef stated that it may have sat out for a while today and that it was leftovers from something. COS- discarded.
  • KDA_4_20211A2 - --Located on a shelf under prep table 2, there were five rubber spatulas that were chipped and one rubber spatula that was badly cracked. COS- discarded.
  • KDA_4_50111B - --The seal of the WIF is torn.
  • KDA_4_60111A - --Located under prep table 1 in the kitchen, there was one plastic food container stored as clean that was soiled with food debris. Corrected on site (COS)- washed. Located on the dish rack next to the three vat sink, there were nine plastic food container lids stored as clean that were soiled with sticker residue. COS- moved to wash area. Located on the prep table next to the dry storage area, there was a meat slicer that was soiled with food debris. The person in charge (PIC) stated that it was last used yesterday. COS- washed.

  • KDA_4_60211E4 - --There is mold on the inside rim of the ice machine.
  • KDA_5_501113 - --The door to the outside dumpster was open.
  • KDA_5_50117 - --The trash can in the unisex restroom was not covered.
  • KDA_6_20214 - --The door on the unisex restroom was not self closing.
  • KDA_6_20215A3 - --There are two 1/4 inch holes at the bottom of the door to the outside at the front of the kitchen.
  • KDA_6_50111 - --The paint on the floor of the kitchen is chipping in places.
  • KDA_6_501112 - --There were several dead bugs on the floor along the wall at the back of the kitchen.
  • KDA_7_10211 - --Located on a rack above the mop sink, there was an unlabeled container of yellow liquid which the kitchen employee identified as lemon degreaser. COS- labeled.
