  • KDA_2_40111 - --Sitting on a ledge of the mechanical ware washing machine, an opened cup with a clear liquid was in it. COS-cup was removed.
  • KDA_3_30212 - --In the pie making area, there was clear container that was labeled mayonnase with out a common name. Employee said it was oil. COS-container was labeled.
  • KDA_3_50116A2 - --Silver King lettuce chopper and cooler, 49.6F, ambient was 54F. Employee said lettuce had been in there since 11:00AM. COS-lettuce was voluntarily discarded. Left side make table reach in,shredded yellow cheese 45.2F and shredded white cheese 44.9F. These items were sitting at the front of the unit by the doors. Employee said the cheese had been in there since about 2:00 PM. COS-cheese was put in freezer to cool. Left side reach in make table cooler, shredded yellow cheese 46F and shredded white cheese 48.4F. These cheeses were sitting in the front of the unit by the door. Ambient was 40.7F Employee said the cheeses had been in there since 11:00 AM. Ambient was 41.2F. COS-cheese was voluntarily discarded. In the two door sliding unit, raw cracked egg in a container dated 1-5-16, 47.5F. Employee said these had not been out. COS- raw egg was voluntarily discarded. Ambient of two door sliding cooler was 42.6F.
  • KDA_4_20211A2 - --On the shelf under the make table behind the cooking area, there was one clear lid that was melted. COS-lid was removed.
  • KDA_4_50111B - --The left door of the two door reach in cooler in the kitchen, does not stay shut. When you shut a door it bounces back open.
  • KDA_4_60111A - --Under the make table behind the cooking area, there was one lid that had sticker residue. On a shelf beside the pop carbonator, there were two lids that had tape on it. COS(corrected on site)-items were removed and placed at three compartment sink.
  • KDA_6_20111 - --The floors in the kitchen area, are rough concrete and have holes in area that make the floor hard to clean.
  • KDA_6_50111 - --There is cracked floor tile through out the kitchen.
  • KDA_6_50112A - --In the women's restroom, there is one stained and one loose ceiling tile. There is six old rodent droppings under the east end of the three compartment sink.
  • KDA_Defacto - --Right side make table top cooler, chopped tomatoes 42F. Right side reach in cooler, chopped ham 42.5F. In the two door sliding unit, left side raw cracked eggs 41.2F. Raw shell egg, internal 44F.

  • KDA_2_30114H - --An employee in the salad preparation area, was wiping down the cutting board with a moist towel and then changed gloves with out hand washing. She then put on new gloves and touched ready to eat grapes. COS-grapes were washed, gloves were changed and hands were washed.
  • KDA_3_30212 - --In the cooling area, a metal with a white powder in it was without a common name. Employee said it was powdered sugar. On a shelf below the table top grill, a container with a white powder in it and a container with a light brown paste was with out a common name. Employee said they were corn starch and gravy base. In the pie baking area, a mayo container with a yellow liquid was without a common name. Employee said it was oil COS-containers were labeled.
  • KDA_3_30412A - --In the server station make table top cooler, two containers of chopped cooked ham had single serve containers with handles touching the food contact surface. COS-single serve containers were removed.
  • KDA_3_30412B - --In the pie baking area, a container of sugar had a scoop handle touching the food. COS- Scoop handle was placed above flour.
  • KDA_3_50116A2 - --Two door in kitchen, cottage cheese 45.9F. Employee said it had been out at 9:00 AM today while they prepared single serve bowls. In the make top cooler, cut honey dew melon 45.8F. Employee said honey dew was cut at 10:00 AM. Left side make table reach in cooler, milk 47.5F. Employee said it was opened at 10:00 AM and was out while gravy was prepared. Ambient of the left side make table reach in cooler was 37.8F. COS-(corrected on site)-Items were left in coolers to cool down.
  • KDA_4_20211A2 - --Under the pie crust make table, there were three plastic lids and one plastic container that had deep cracks and corners missing. COS-containers and lids were discarded.
  • KDA_4_204112B - --In the back storage room, the sliding two door reach in does not have a thermometer.
  • KDA_4_50111B - --The left door of the two door reach in cooler in the kitchen, does not stay shut. When you shut a door it bounces back open.
  • KDA_4_60111A - --Sitting on a shelf under the make table with the slicer, there were three plastic green lids with tape on them. On a shelf under the make table in front of the three door stainless steel reach in, there were one lid that had black debris on it. Sitting in a red basket by the pop carbonator, three plastic lids with tape on them. COS-all items were placed at ware washing machine. The silver King lettuce shredder has tape on the food contact surface.
  • KDA_4_60111C - --The grill hood, has an accumulation of grease. In the pie baking area, the large containers of flour and sugar have dried on food debris on the lids and outside of the containers. The container that is storing clean lids under the slicer has food debris that is coming in contact with the food contact surfaces. On top of the two door reach in, a free standing fan has an accumulation of dust. The handles on the three door stainless steel reach in, have an accumulation of dried on food debris. A brown cart located next to the stainless steel reach in with pies cooling on it, has an accumulation of black debris, dirt, food and dried on food debris.
  • KDA_4_60211E4 - --The area around the pop nozzles in the pop machine in the kitchen and in the server station, have dried on debris. The ice machine in the server station, has brown and black mold on the deflector.
  • KDA_4_60213 - --The inside of the three door stainless steel reach in, has an accumulation of food and white powder on the walls, shelves and floor of the unit. The tracks on the two door cooling units in the server station and back storage room, have an accumulation of food debris.
  • KDA_6_20111 - --The floors in the kitchen area, are rough concrete and have holes in area that make the floor hard to clean.
  • KDA_6_20213B2 - --There is a insect control device on a south wall, that is located over an open container of Italian seasoning and an open container of basil leaves. There are also five closed bottles of seasoning below it. COS-seasoning bottles were removed.
  • KDA_6_50111 - --There is cracked floor tile through out the kitchen. In the women's restroom, there is one stained and one broken ceiling tile. In the salad preparation area, there are two light that are out. There is still enough light to do the job safely.
  • KDA_6_501111A - --There are three new rodent droppings under the three compartment sink. Establishment has licensed pest control, that was out two weeks ago.
  • KDA_6_50112A - --Under the three compartment sink, there are about 50-75 old rodent droppings. The walls behind the table top grill and the shelf under the table top grill have an accumulation of food debris. Behind the pop carbonator, there is black mold on the wall. There is dirt and dried on food debris on the wall to the left of the stainless steel reach in. In the server station, there is dirt and trash to the left of the make table. In the server station, there is mold on the wall to the right of the ice machine. The ceiling through out the establishment has an accumulation of dust. Next to the stainless steel reach in, the shelf has an accumulation of dried on ketchup.
  • KDA_6_50118 - --The hand washing sink near the grill area, has an accumulation of dried on debris.
  • KDA_7_10211 - --On a shelf under the server computer, a spray bottle with a blue liquid in it was with out a label. Employee said it was sanitizer. COS-bottle was labeled.
  • KDA_7_20111B - --Sitting on the ware washing machine, opened bottles of Mach turbo detergent and a bottle of Mach turbo dry mate plus. There was also clean drinking cups sitting on a shelf below the chemicals.
  • KDA_7_20911 - --In the salad preparation area located on a food storage rack with bread and single serve items, were a purse and cell phone.
  • KDA_Defacto - --Chicken on the grill, 184.8F. --cooling is done in a ice bath. --Frozen food is frozen. --In the three well steam table, made on site chili 143.6F. --Mechanical ware washing machine, 158.3F. --Silver King, lettuce 43F. Pie case reach in, 41.7F. Two door in kitchen, raw egg 45.2F. Three door stainless steel reach in, corn dog 33.8F. Two door reach in, raw cracked eggs 42.9F. Ambient of two door reach in 42.6F. In the server station, sitting in a pan of ice shredded yellow cheese 41.9F, make table top cooler, chopped tomatoes 40.3F, cooked chopped chicken 42.4F. Left side of make table top cooler, salsa 37.2F. Right side make table reach in cooler, chopped cooked chicken, 41.5F. In the storage room, the two door reach in ambient was 36.4F.
