- KDA_3_30212 - --Located on the bottom shelf of a prep table next to the wok in the kitchen, there was an unlabeled container of white powder which the PIC identified as salt. COS- labeled. Located on the prep table next to the grill, there were two unlabeled containers of white powder and an unlabeled container of black powder which the PIC identified as sugar, salt, and pepper. COS- labeled.
- KDA_3_30511A1 - --There was a container of battered chicken stored on the floor of the WIF.
- KDA_3_50116A2 - --Located in MT 2, there was a container of cooked noodles at 48F, a container of house made red curry sauce at 53F, a container of house made yellow curry sauce at 52F, a container of house made drunken sauce at 51F, a container of house made pad thai sauce at 53F, and a container of house made lemon sauce at 53F. In the RIC 2 under MT2, there were two containers of pad thai noodles at 55F. Person in charge (PIC) said that they just restocked the MT at 1:00 this afternoon. Corrected on site (COS)- put all potentially hazardous foods on ice.
- KDA_4_30111 - --The ambient air temperature of the MT/RIC unit #2 was 49.6F. COS- all potentially hazardous foods were put on ice. PIC said he has a new MT that he will use to replace this one, and it should be here by the end of the day.
- KDA_4_50111B - --The seals of the RIC 2 under MT 2 are broken.
- KDA_4_60111A - --Located on a shelf at the back of the kitchen, there were 14 metal food containers stored as clean that were soiled with food debris. COS- moved to wash area.
- KDA_4_60111C - --There is grease on the sides of the fryer and the wok. The fan guards of the WIC and WIF are dusty.
- KDA_4_60211E4 - --The inside ceiling of the ice machine had an accumulation of mold on it.
