KDA_3_50118A2 - --There were opened gallons of skim milk, 2% milk and whole milk in reach in cooler 1 at the front counter without any date marking. PIC stated all three containers were opened yesterday 1.25.16 more than 24 hours prior. COS- PIC correctly date marked.
KDA_4_60111A - --The holder for the ice dispensing scoop for the bulk ice unit has standing water and dirt build up along the inside surface. COS- PIC cleaned the holder and the scoop.
KDA_5_50116B - --There was no trash receptacle available at the handsink in the back room where refuse is generated.
KDA_Defacto - --chili 165 (steam table) --Hot water measured at 162.5F in the dishmachine. 3 vat not set up, PIC described process. There were QT test strips available (expiration 7/2016). There were two alternate QT test strips in use expiring 2014 and 2015, PIC voluntarily discarded both. --philly sandwich 38 (walk in cooler) milk 42 (reach in cooler 1 front) milk 42 (reach in cooler 2 front) club sandwich 41 (reach in cooler 3 front) 1/2 & 1/2 40 (canister in lobby) --There is a pest control company contracted, no activity. --There were opened gallons of skim milk and 2% milk in reach in cooler 2 at the front counter without any date marking. PIC stated both milk containers were opened today 1.26.16. Educated staff on proper date marking procedures. --Wiping cloth QT measured at 200ppm, test strips provided.
