The kitchen employee said that these items had been cooked on 1/23. COS- date marked. Located in the slide RIC, there was a container of cooked chicken, an opened package of deli ham slices, and an opened package of deli turkey slices with no dates marked on them. The kitchen employee said that the chicken was made on 1/22 and the deli meats had been opened on 1/21. COS- date marked. Located in the Kenmore RIC, there was a container of cooked chili at 49F with no date marked on it. The kitchen employee said that it had been made 1/20. COS- discarded since it was out of temperature.
  • KDA_4_30111 - --The ambient temperature of the Kenmore RIC was 45.5F. COS- discarded all potentially hazardous foods that had been in the cooler overnight, moved the food items that had been cooked today to the slide RIC. Turned the knob on the RIC to turn the temperature down.

  • KDA_4_60111A - --Located on the dish rack next to a freezer, there was one plastic food container lid stored as clean that was soiled with sticker residue. COS- washed.
  • KDA_4_60111C - --The fan guard of the slide RIC is soiled with dust. The tracks of the slide RIC are soiled with food debris. There is grease on the sides of the grill and fryer. There is grease on the inside of the fryers. The vent hood above the grill and fryer is soiled with grease. The vent cover on the ceiling above the prep area of the kitchen is soiled with grease and debris. The fan guards of the bar area WIC are soiled with dust.

  • KDA_4_90311A - --Located at the bar area next to the ice machine, there was a box of single use cups stored on the floor.
  • KDA_6_30112 - --There were no paper towels available at the bar hand sink.
  • KDA_6_50111 - --There were several missing ceiling tiles in the kitchen. There were several stained ceiling tiles in the kitchen. There is no sheetrock at the bottom of the wall behind the water heater in the storage room. There is exposed ceiling in the storage room by the kitchen.
  • KDA_6_50112A - --There is an accumulation of (100+) dried rodent droppings on the floor along the back wall of the kitchen prep area, behind the grill, behind the fryers, and on the floor and wall of the storage room. The establishment has a pest control operator and showed documentation. The last visit by the pest control company was on 1/4. The wall and floor behind the grill and fryers is soiled with grease and debris. There are 20 dried rodent droppings on the shelf under the main prep table. There are 40 dried rodent droppings on the floor behind the ice machine at the bar area. There is broken glass on the floor of the bar area walk in cooler (WIC).
  • KDA_7_10211 - --Located at the front of the bar, there was one unlabeled spray bottle of green liquid which the person in charge (PIC) identified as simple green cleaner. COS- labeled.
  • KDA_7_20111A - --Located on top of the Frigidaire reach in freezer, there was a bottle of isopropyl alcohol stored next to a bag of flour. COS- moved alcohol.
