Above the prep sink, stored as clean were two lids that had dried on food debris and two that had sticker residue on them. COS-person in charge removed all items for rewashing. COS- removed tomatoes KDA_3_30211A1b - --In WIC there was fully cooked RTE bacon pieces stored next to raw bacon. COS- moved RTE bacon pieces KDA_3_30412B - --In the large ice machine in kitchen the handle was stored in contact with ice. KDA_4_20211A2 - --In the dry storage area there were 5 metal food containers that were cracked or dented. COS- discarded KDA_4_50111A - --In the kitchen above prep line there was a microwave that had the inside door of peeling and the upper shield broken. KDA_4_50112 - --In the kitchen there is a cutting board that is deeply scored and stained. KDA_4_60111A - --In the kitchen above prep table there were 5-10 plastic food containers stored as clean but had dried food debris. COS- removed for cleaning In kitchen there was a lettuce slicer stored as clean but contained dried food debris On the make table there was a container with 5-10 utensils stored as clean but had dried food debris. KDA_4_60111C - --There is an accumulation of food debris and grease residue on shelving throughout the kitchen. KDA_4_90111B - --IN the dry storage area there was 10 plastic food containers stored as clean but stacked wet. On a shelf above food prep table there was 5 plastic food containers stored as clean but stacked wet. COS- removed KDA_4_90311A - --In the storage shed there was 2 boxes of single use cups stored on the floor. COS- moved cups off floor KDA_5_20515B - --The cover is missing from the backflow prevention device at the mop sink. KDA_6_20215A1 - --There is a hole in the outer wall of the storage shed. KDA_6_20215A3 - --The back door by the mop sink has a 1/2 inch gap around the bottom. KDA_6_30112 - --The hand sink in the back kitchen did not have paper towels. COS-paper towels provided KDA_7_10211 - --In the kitchen there was 1 container of clear liquid that PIC identified as sanitizer not labeled. In the kitchen there was 1 container of blue liquid that PIC identified as pantastic not labeled.COS labeled KDA_7_20111A - --In wait station there was a container of sanitizer stored next to plates.
