COS Items were discarded. KDA_4_60111A - --There was a plastic food grade container stored as clean on a shelf above the three vat sink that had sticky residue left on the container form a previous us. COS Item was placed in the three vat sink for cleaning. There was a metal food grade pan being stored on a shelf above the three vat sink that had dried food debris on the inside of the pan from a previous use. COS Item was placed in the three vat sink for cleaning. There was a rubber spatula stored as clean hanging from a hook next to the three vat sink with a hair on the spatula. COS Item was placed in the three vat sink for cleaning. KDA_6_30112 - --There was no paper towels in the dispenser located at the hand sink in the back kitchen area. COS Paper towels were placed in the dispenser. KDA_6_30114 - --There is no hand washing signage posted in the men's restroom. KDA_6_501111A - --There are eight live small flies on the wall behind the clean out sink located on the front serving line. There are 40-50 live small flies on the walls, cardboard boxes, and coming out of the dirty towels bag. PIC said they have a pest control company. Paper work was provided. Last service was on 9/24/2015 KDA_6_50112A - --There is dried grease on the walls, ceiling, and floors located in the kitchen area. KDA_7_20111B - --There was a container of sanitizer solution with towel being stored on top of a food prep table were ice cream blizzard's are made. COS Chemical was removed. There was a bottle of ice machine cleaner being stored on top of the ice machine. COS Chemical was removed. KDA_8_30411A - --Current 2015-2016 license was posted but not in public viewing.
