KDA_3_30212 - --Under prep table near the fryers, there was a white container with a white substance in it with out a common name. Person in charge said it was salt. Back by the hot water heat there were two white containers without a common name. Person in charge said they were dry beans and rice. COS-containers were marked with common name. KDA_3_30412B - --In the ice machine near the bar area and in the kitchen area the ice scooped was stored in the ice with the handle touching the ice. Under the make table on a shelf, in a container of salt and corn meal there were bowls with out handles touching the food. COS-bowels and scoops were removed. KDA_3_50114A2 - --Sitting on a make table in a container of ice, made on site salsa at 45.4F. Person in charge said the salsa was made last night at 11PM. COS-salsa was discarded. KDA_3_50116A2 - --In the west end make table: Made on site green sauce 49.5F, sliced raw chicken 51.7, raw chicken 55F, raw beef slices 55.4F, raw shrimp 55.4F. Raw thinly sliced steak 54.3F. Person in charge said these items had been in make table since 10:30AM today. COS-small quantifies were put on ice and the rest were put in the walk in cooler. There were three raw and thawed tilapia filets 53.7F, 54.3F, 53.8F, one package of imitation crab 52.6F. Three raw hamburger patties(Person in charge discarded with out temperatures being taken). Person in charge said these items had been in the make table since yesterday, and he didn't know when it was last in temperature. In the reach in make table: black beans 54.2F, chorizo 51.7F, 51.4F,51.6F,53.4F.(person in charge said, no more temperatures). Person in charge said these items had been in there since yesterday. COS-items were discarded. KDA_4_30111 - --The west end make table was not holding at PHF at 41F. KDA_4_60111A - --In the ware washing area, stored as clean on a shelf were five metal pans with sticker residue on them. COS-pans were moved to three compartment sink. KDA_4_90311A - --Above the ice machine, there are two ice buckets and two ice scoops that are stored on top with ice contact facing up. COS-items were moved to a lower shelf. KDA_6_20213B1 - --There were three sticky fly traps over the make tables and grill areas in the kitchen. COS-traps were removed. KDA_6_20215A3 - --The North side front door has about 1/2" gap along the bottom left edge. KDA_6_50111 - --In front of the walk in cooler and ice machine, there is cracked floor tile. There are holes in the east kitchen wall and around the hot water heater walls. There is floor tile missing around the hand washing sink near the ware washing area. There is one broke ceiling tile in the restroom hallway. In the dishwashing area, there is baseboards missing. in the kitchen there is one ceiling tile that is being held up with a piece of wood. KDA_6_501114B - --Out the back door, there is food and garbage on the ground that is drawing flies. KDA_6_50112A - --There are stains through out the kitchen area on the ceiling tile. In the rest room hallway there are two stained ceiling tile. In the bar area, there are three stained ceiling tiles. Above the third booth on the left side there is one stained ceiling tile.The base boards and the floors around the base board have an accumulation of dirt. Behind the three compartment sink, there is black mold on the base board. In the bar area, the True fan covers on the coolers, have an accumulation of dust. In the bar area, the ceiling vents have dust. In the kitchen area, the light covers have an accumulation of dust.
