KDA_3_50119B3 - --Located on the kitchen make line, there were sandwich prep trays of sliced tomatoes, shredded cheese and cut lettuce that had use by 12:21pm on 9/1 stickers on them. Person in charge (PIC) indicated that they had replaced the food items in these trays since 12:21pm but did not know when. COS- discarded. KDA_4_20211A2 - --Located in a container on the utensil storage shelf in the kitchen, there was one plastic scoop that was pitted. COS- discarded. Located on the clean dish storage rack at the back of the kitchen, there were three plastic food container lids that were cracked and one plastic food container that was cracked. COS- discarded. KDA_4_60111A - --Located on the shelf under the sandwich make station, there were six plastic food containers stored as clean that were soiled with dried food debris. Corrected on site (COS)- moved to wash area. Located in a container on the utensil storage shelf next to the tea maker in the kitchen area, there were two metal scrapers and one plastic spoon stored as clean that were soiled with dried food debris. COS- moved to wash area. Located on the utensil storage shelf in the kitchen area, there were two plastic food containers stored as clean that were soiled with dried food debris and one plastic food container lid stored as clean that was soiled with sticker residue. COS- moved to wash area. Located on the dish storage shelves in the back of the kitchen, there were five plastic food container lids stored as clean that were soiled with sticker residue and two plastic food containers stored as clean that were soiled with dried food debris. COS- moved to wash area. KDA_4_60111C - --Located in a container on the utensil storage shelf in the back of the kitchen, there was a bucket opener that was soiled with dried food debris. COS- moved to wash area. KDA_4_60211E4 - --There is mold on the inside of the ice machine under the rim in the back of the kitchen. There is mold on the south side ice chute on the customer self serve soda fountain in the lobby. KDA_4_90311A - --There was a box of single use drink cups stored on the floor in the employee break room. Located on the utensil storage rack at the back of the kitchen, there were two storage containers that had food debris in them that were being used to store clean utensils. KDA_4_90311B - --Located on a shelf under the kitchen make line, there were six plastic food containers stored as clean that were stacked wet. COS- moved to wash area. KDA_5_20515B - --The hand sink on the east side of the kitchen make line and the east hand sink in the women's restroom were slow to drain. The east hand sink faucet in the women's restroom was broken. KDA_6_20213B2 - --There were fly traps located above the French fry hot holder at the front of the kitchen, above a kitchen prep table towards the back of the kitchen, and above the clean dish storage rack in the back of the kitchen. KDA_6_50112A - --There is trash and debris on the floor and along the walls in the storage areas of the kitchen. KDA_7_10211 - --Located on the counter in front of the Frappe machine, there was an unlabeled container of pink liquid that had spoons stored in it. PIC indicated that this was sanitizer water used to soak the soiled spoons. COS- dumped container and refilled with regular water for soaking. KDA_7_20111A - --Located on the storage shelves in the back of the kitchen, there were boxes of floor cleaner stored in contact with boxes of kitchen gloves. COS- moved. KDA_7_20111B - --Located on the storage rack in the back of the kitchen, there were containers of paint and a bottle of WD-40 stored above a shelf with boxes of dill pickle slices, mustard pouches, strawberry shake syrup, and sweet chili sauce pouches. COS- moved. Located on the front counter in front of the Frappe machine, there was a container of sanitizer water used for soaking soiled spoons stored above containers of flavored syrups. COS- emptied sanitizer water and refilled with regular water for soaking the spoons.
