4-302.14 - --Facility's CL test strips expired on 05/01/2014. Facility did not have any other test strips. Educated PIC on the need to purchase new test strips.
KDA_2_30114F - --Employee took out the trash from the inside dumpster to the outside dumpster and did not remove gloves and wash hands before beginning to handle a basket of chips. COS by educating the employee on the need to wash ands between tasks. Chips were VD.
KDA_2_30115 - --Employee washed hands in the mop sink. COS by educating the employee on the need to only wash hands in the hand washing sink.
KDA_3_50111 - --RIF was not keeping food frozen. PIC said that the facility was going to get a new RIC and RIF and keep the food products. Items were at above 41 and PIC COS by VD.
KDA_3_50116A2 - --RIF: Chili Rellenos 46.2. Other raw beef items were not frozen. Ambient Air temperature 60.2. PIC COS by VD items.
