PIC said that they were prepared or cooked on Saturday. KDA_4_10111A - --Ice stored inside cooler in a plastic container that is not food grade. KDA_4_20211A2 - --Crock Pot had visible crack on the surface and was not free of breaks and imperfections. KDA_4_60111A - --Crockpot and utensils stacked and stored as clean had food debris. PIC removed items to be washed. KDA_4_60111C - --Shelving above the prep tables and MTs had visible grease and debris accumulation.
