KDA_3_30412B - --A single-serve portion cup was stored in a container of salt. KDA_3_50114A1 - --Establishment made tamales 99F were resting in a bowl next to the front counter. PIC stated the tamales were cooked at 11:00 AM 12-14-15. PIC stated the tamales were cooling. COS, tamale placed back into a steamer for reheat to 165F. PIC not sure when the tamales went below 135F. Inspector provided cooling procedures to the PIC. KDA_3_50118A1 - --Cooked beef was in the make table with a preparation date of 12/7, Deli ham was in the make table with an opened container date of 12/7/. The food items were held more than 7 days. COS, items discarded. KDA_3_50118A2 - --A container of cooked pork was in the make table without a preparation date on the container. PIC stated the pork was cooked on 12/12/15. Containers of green salsa were in the make table without a preparation date. PIC stated the salsa was made on 12/12/15. COS, all food items dated. KDA_4_20211A2 - --Cooked pork was stored in a plastic container with a crack in the plastic lid cover. KDA_6_30111 - --The electric hand soap dispenser does not dispense soap. PIC stated the battery must be low. COS, provided a hand soap. KDA_6_50112A - --Old mouse dropping were behind the reach in freezer. KDA_5_20515B - --The faucet of the hot water at the handsink in the food prep area does not work without. The hot water has to turned by valve under the handsink. KDA_6_20215A3 - --The back outer door has 1/4 inch gap space under the door. The space is large enough for pest to enter the establishment. KDA_6_501111A - --15 fresh mouse dropping were under and behind the food storage rack in the food prep area. 8 fresh mouse droppings were in a cabinet space on shelf next to food contact equipment. 20 fresh mouse droppings were in a cabinet space in the stairway going into the basement area. 30 plus fresh mouse droppings were the basement food storage, and single serve items were stored.
