- KDA_3_50118A1 - --In the walk in cooler, the following items were dated and held for more then seven days: made on site ham and beans, dated with a discard date of 1/8. Cooked on site taco meat, discard date was 1/15. Made on site white gravy, with a discard date of 1/13. cooked and made on site enchilada's, with a discard date of 1/17. COS-all items were voluntarily discarded.
- KDA_4_20211A2 - --In the kitchen on the knife strip, there was a knife stored as clean with the handle melted. On the metal rack in the kitchen, stored as clean was one container that was pitted along the food contact surface. COS-items were discarded.
- KDA_4_204112B - --There are no thermometers in the reach in cooler in the bar or the Hot point reach in cooler in the kitchen.
- KDA_4_60111A - --Stored as clean on a metal shelf in the kitchen, two containers and one lid that had sticker residue. Stored as clean on a knife strip in the kitchen, one knife that had dried on food debris on the blade near the handle. COS-all items were placed for rewashing.
- KDA_4_60211E4 - --The ice machine, has a small amount of brown mold on the right side of the plastic deflector and wall.
- KDA_4_90311C - --In the kitchen stored on a rack above the make table, the single serve items are open and not protected from contamination.
- KDA_4_90411A - --There is a container of serving utensils that have the food contact surface facing up, so they will be touched. COS-utensils were placed the other way.
- KDA_6_20215A3 - --The front door has about 1/4" gap along the bottom.
- KDA_6_30112 - --There were no hand drying towels at the bar hand washing sink. COS(corrected on site)-towels were placed at hand washing sink.
- KDA_6_30114 - --The following areas did not have hand washing signs, men's and women's restroom, hand washing sinks in the bar and in the kitchen area. COS-hand washing signs were left.
