- KDA_3_30414B1 - --There was one wet wiping cloth sitting on the counter in the bar area and one wet wiping cloth sitting on the cooking area counter. The wiping cloths were not in the sanitize solution.
- KDA_3_50116A1 - --Located at the make table there was one bowl of cooked chicken sitting at a temperature of 122F. The cook then added new cooked chicken on top of the out of temperature chicken. COS. PIC destroyed chicken and made fresh batch.
- KDA_3_60311A - --Menus do not have the undercooked food items identified. Menus do have the disclaimer "consuming raw or soft boiled, braised eggs may increase your risk of food-borne illness". COS. PIC marked each menu with a marker so that the foods are now identified.
- KDA_4_20211A2 - --Located on the shelving for clean dishes, there were 3 plastic food containers with cracks and melted areas and are not cleanable. COS. Items were removed from shelving to be destroyed.
- KDA_4_60111A - --Located on the shelving for clean dishes there were 2 pans with duct tape on the outside. COS. Pans were placed at dish area to removed tape and rewash. Educated staff.
- KDA_4_60111C - --The vent hood area and filters have an accumulation of grease debris.
- KDA_4_90311A - --Located in several locations in the FE there were 5 cases of single use items sitting directly on the floor. 1 case each: napkins, bowl, cups, chopsticks, forks.
- KDA_5_20511B - --Located at the hand sink in the upstairs area, the sink had a drinking glass inside and ice had been recently dumped. PIC said they use it for hand washing and for dumping drinks. COS. Educated staff.
- KDA_5_20515B - --The hand sink in the cooking area has a constant drip from the faucet.
- KDA_6_30311A - --Located above the rack for clean dishes in the dish area and above the cooking stove there are several lights that are not working.
- KDA_6_50112A - --The floor areas in the bar and cooking area and under equipment have an accumulation of black sticky debris. The storage room under the stairs has an accumulation of clutter and it is not possible to clean the floor or look for pest activity.
- KDA_7_10211 - --Located on the shelf in the cooking area there was one red bucket with liquid inside and no label. PIC said it was sanitizer solution for wiping cloths. COS. Bucket was labeled.
- KDA_7_20111A - --Located on the shelf in the cooking area there was one bucket of sanitizer sitting beside 1 pan of dry noodles. COS. Chemical was moved.
- KDA_7_20111B - --Located on shelving directly above the clean sanitized dishes and the in use sanitize solution for clean dishes, there were 14 gallons of sanitizer and 4 gallons of bleach, and 1 bottle of hand sanitizer. COS. Chemicals were moved. Educated staff.
