KDA_3_50116A2 - --A container of soup made with pork 55F, and one made with goat 59F were stored on top of table in the food prep area. PIC stated the food items were taken out of the reach in cooler this morning around 7 AM. COS, soups discarded.
KDA_4_20211A2 - --Red pepper sauce was stored in plastic food storage container with large crack down the side of the container. COS, red sauce removed.
KDA_6_20215A1 - --The front door has 1/42 gap space around the opening and bottom of the door. The space is large enough for pest to enter the establishment.
KDA_6_50111 - --Over 9 ceiling tiles in the food were water stained. 10 water stained ceiling tiles were in the second dinning area. 8 water stained ceiling tiles were in the food prep area.
KDA_6_501111A - --1 live roach moving was on a glue trap next to a recently deceased mouse. The glue trap was located under the (P) reach in cooler in the food prep area. 20 fresh mouse droppings were behind a reach in freezer. 20 fresh mouse droppings were on the floor behind the front register area. 1 fresh mouse dropping was on a plastic bag that contained bread. The plastic bag with bread was stored on storage rack in the food prep area. PIC provided pest control invoice with a service date of 12-2-15.
KDA_6_501112 - --Dead roaches and roach egg sacks were on top of the paper towel unit in the food prep area.
KDA_7_10211 - --A spray bottle with a purple liquid was hanging under a handsink without a common name on the working container. PIC stated liquid in the spray bottle is for cleaning the tables.
