KDA_3_60211B1 - --There was a container of yellow substance with a label that said "cornstarch". PIC said it was actually "cornmeal". COS. Label was corrected.
KDA_4_20211A2 - --Located on the shelving for clean dishes, there was one plastic container out of 10 that had a 3 inch crack on the edge. COS. Item was placed in trash can.
KDA_4_60111A - --The table mounted can opener blade had dried food debris on the food contact surface. PIC did not know when it was last washed. COS. Item was washed. Located on the shelving for clean dishes, there were 4 out of 10 plastic containers with tape residue. COS. Tape was removed and items were rewashed.
KDA_4_60111C - --The bottom shelving of the RIC has an accumulation of dried food debris. The utensil drawer exterior and interior has an accumulation of dried food debris. The dish sink table has an accumulation of dried food debris on the bottom shelf. The RIC doors and door handles have an accumulation of sticky debris.
KDA_4_90311B - --Located n the shelving for clean dishes, there were 5 pans that were stacked together wet.
KDA_5_20515B - --The kitchen hand sink drain is very slow to drain the water. The water will over flow if you leave the faucet on too long or if multiple people wash their hands at once.
