KDA_3_30414B1 - --The sanitizing buckets had 0PPM quat. When employee re-made the sanitizer, the premixed quat was not working. The person in charge fixed the tubing. COS-buckets were remade and corrected to 300PPM. KDA_3_30511A1 - --In the walk in cooler, there was an open box of lettuce with five packages in it sitting on the floor. COS-lettuce was moved to a shelf. KDA_4_50111A - --The walk in cooler, has ice accumulation on the door jams, ceiling, walls. KDA_4_50111B - --The latch on the walk in cooler and the walk in freezer are broken. The person in charge said a new latch is on order for the walk in cooler. KDA_4_60111A - --On a shelf in the ware washing area, stored as clean were four containers with sticker residue, one container had dried on food, one had dried on food on the bottom surface,three were greasy to the touch. COS- person in charged discarded three containers and the rest were put at the three compartment sink. KDA_4_60111C - --The fans in the walk in cooler have an accumulation of dust. KDA_4_60211E4 - --The pop machine at the drive through, has black mold up behind the nozzles and at the base where it meets the drain rack. The racks on the pop machine in the dining area have black mold on them. KDA_4_90311B - --Back in the ware washing area, there were 41 containers stacked wet. COS-four of the containers were discarded, the rest were put at the three compartment sink. KDA_6_20113B - --There is water sitting along the base boards on the south side of the food prep line and in the back storage room around hot water heater. Person in charge said, that when they mop, the water does not come out of the tile and it sits, due to the grout that is missing. KDA_6_50112A - --The ceiling through out the establishment has an accumulation of dust. KDA_7_20111B - --Hanging with nozzle hooked onto a rack under the right cash register, was a spray bottle of bathroom cleaner with single serve cups stored beneath it. COS-cups were moved to another location. KDA_Defacto - --Frozen food frozen. --In the stainless steel two door warmer, refried beans were 157F, North side of food line: taco meat 157.5F, south side food line: chicken 171.3F. --North side food line: make table shredded cheese 39.2F, reach in make table, shredded cheese 41.3F.
