  • R/- Damaged or lacking thermometer in the refrigeration units under the cappuccino or smoothie machines, but these units were holding at 41°F and below.
  • - Ice accumulation on the ceiling and floor in the walk-in freezer
  • - Wall tiles by the hand sink and three vat sink
  • - Air curtains in the walk-in freezer
  • - Damaged insulation and heat tape surround in the condenser line in the walk-in freezer
  • - Stained ceiling tiles in the storage area
  • - Lacking light shield in the storage
  • - Door seal on the Traulsen freezer
  • - Lacking grout between the floor tiles in the drive thru area
  • Repair these areas to provide a cleanable surface.

  • - Floor surface in both walk-in units
  • R/- Soiled door seal drive thru Beverage Air unit
  • - Equipment rack by the walk-in cooler

  • 6.2 Observed the hand washing signs were lacking in the women's public restroom. Provide this.
  • 8.1 The log for the heat dispensing machine is incomplete. Be sure to complete the log daily with the required information.
  • Note: This operation did submit plans to this office for a remodel of the dining room area, and relocation of equipment on the grill line.

  • 4.5/4.6 Observed the wedger, the pop nozzle on the drive thru machine, ice machine chute and surrounding area were soiled, and/or contained a mold-like growth. Food contact surfaces shall washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • *3.4 Observed the shredded cheese was holding at 49°F, the corn and bean salsa was holding at 49°F, Sure shot machine in the
  • Drive thru area contained an air temperature of 52°F, and the product was holding at 50°F. Food and refrigeration units shall be maintained at 41°F and below to limit the growth of organisms.
  • 6.4/4.4/4.1 The following areas were damaged:
  • - Exterior of the iced coffee dispenser are damaged/uncleanable
  • - Damaged thermometer in the Sure shot machine counter unit, but this unit was holding at 41°F.
  • Repair these areas to provide a cleanable surface.
  • R/4.5/6.4 Clean the following areas:
  • - Soiled door seal drive thru Beverage Air unit
  • - Ice accumulation on the air curtains, and product stand in the walk-in freezer
  • 4.1 Provide a lacking thermometer in the Smoothie and cappuccino units, holding at 41°F and below.
  • 3.2/4.8 Observed a case of opened pancakes and a refrigeration rack was stored on the floor. Food and equipment shall be stored six inches off the floor to facilitate cleaning and to prevent contamination.
  • 3.2 Store wet wiping cloths in sanitizing solution between uses to limit the growth of bacteria.
  • 3.5 Observed the bagged ice was lacking the name of the food, a weight, and a manufacturers name and address. Provide this.

  • 2.4/4.1/4.4 The digital thin probe thermometer was in disrepair. Provide an accurate digital thin probe thermometer accurate within +/- 2°F to monitor temperatures of potentially hazardous foods.
  • C/*4.4/7.0 Observed a squeeze bottle was unlabeled, and contained 0ppm of sanitizer. Provide 50-200 ppm of chlorine sanitizer solution and properly label this bottle in order to avoid confusion and misuse.
  • 6.4/4.4 The following areas were damaged:
  • R/- Lacking grout between floor tile throughout as needed
  • - Exterior of the iced tea dispensers are damaged/uncleanable
  • - Door seal on the burger unit by the clam shell grill
  • Repair these areas to provide a cleanable surface.
  • R/4.5/6.4 Clean the following areas:
  • - Soiled door seals on the refrigeration units as needed
  • - Bottom of the four-door freezer and refrigeration units
  • - Floor in the walk-in freezer unit
  • - Sausage draining equipment and surrounding area
  • - Equipment cart was soiled.
  • 6.4 Observed floor surface under the flat griddles and fryers contained grease accumulation, and the floor surface in the freezer is soiled. Clean these areas.
  • 4.8 Observed there was a stack of containers in the storage room, and the sausage gravy containers by the sausage draining station were stored upright. Invert the storage of containers to prevent contamination of the food contact surface.
  • 3.5 Observed the bagged ice was lacking the name of the food, a weight, and a manufacturers name and address. Provide this.
  • 3.2 Observed a bag of ice was stored in the ice machine. Do not store bags of ice in the ice machine.
  • 4.1 Provide a thermometer in the cooler with parfaits behind the front counter, and the cooler under the coffee machine, currently holding at 41°F and below.
  • Notes: Operation added three Henny Penny fryer model number LOV-FFF, and a Hobart oven .

  • 4.5/4.6 Observed the wedger, ice shield on the ice machine, and the ice chutes on the pop dispensing machine in the dining room were soiled. Food contact surfaces shall washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • 6.4 The following areas were damaged:
  • R/- Lacking grout between floor tiles by the three vat sink and mop sink
  • Repair these areas to provide a cleanable surface.
  • R/4.5 Clean soiled door seals on the Traulsen 4-door unit, Beverage air drive thru unit.
  • 6.4 Floor by the ice machine is soiled, floor surface under the flat griddles contains grease accumulation, floor/wall junctures in the drive thru were soiled. Clean this area.
  • R/C/R4.1 Provide a thermometer in the Sure Shot machine currently holding at 40°F.
  • 4.5 Observed beverage nozzles where they attached to the unit is soiled or contained a mold-like growth, a spill in the cabinet under the pop dispensing machine, exterior of the garbage can in the preparation area is soiled, and exterior of the pop nozzles were soiled. Clean these areas.
  • 4.4 Replace the damaged cheese container, and cracked brewing basket on the Bunn Tea brewer, to provide a cleanable surface.
  • 4.8 Observed the open sleeve of salad container and an open case of coffee liners were stored on the floor. Store equipment six inches off the floor to facilitate cleaning and to prevent contamination.
  • 4.5 Observed an excessive amount of ice accumulation surrounding the delivery door, floor ,ceiling, and walls in the walk-in freezer. Clean or repair this area.
  • 4.2 Provide a QAC test kit to assure proper concentration is maintained.
  • 4.5 Vent on the Toaster is dusty, interior of the milk refrigerator contains a spill, and there was a spill under the Taylor Frappe unit.
  • Clean these areas.
  • 3.2 Label the water squeeze bottle with the name of the food.
  • 4.4 Repair the damaged door seal on the on the one door freezer unit, to provide a cleanable surface.
  • Notes: Operation added Taylor Frappe Machine. No make and model number was able to be obtained.
  • Power outage was experienced by the operation yesterday from 1:00pm-3:00pm. Operation was closed during time frame.
  • This operation recently did have a complaint regarding the hot water in the restroom. Hot water was at 100°F in the restroom today after one minute of running, but did not maintain that temperature after a ten minute period. Recommend to make any needed repairs to assure 100°F water is maintained at the hand sinks to promote hand washing and to prevent food-borne illness.
  • Spoke to maintenance man today regarding, providing hose bibb for the hose when attacked to the mop sink.
