  • 3.4- Chicken strips holding between 119°F-123°F. Maintain potentially hazardous foods hot at 135°F and above to prevent bacteria growth. Chicken was discarded during this inspection.

  • C/R/*7.1 Observed Ajax cleaner and degreaser stored above plastic silverware and hanging on the dipper well on the front freezer. Store toxic chemicals separate from and below food and equipment in order to prevent possible contamination.
  • C/*3.2 Observed 2 chocolate topping and 1 waffle cone dip cans with dented seals on the can rack. Store dented canned goods in a separate designated area for return/credit. Do not use canned goods that are dented on the seal in order to prevent possible contamination.
  • R/3.5 Provide ingredients on the labels for the M &M and Cookie Dough ice cream cake logs and the birthday cakes in the selfserve freezer.
  • R/5.3 Provide an indirectly drained food preparation sink for washing produce.
  • 6.4 The following areas were soiled:
  • -R/chocolate spill and food debris on the floor in the walk-in cooler under the shelving
  • -R/food debris on the floor below the soft serve ice cream units
  • -food debris under the cooking equipment
  • -pop syrup spill under the pop syrup rack
  • Clean these areas in order to maintain physical facilities clean.
  • R/6.4 Observed water damaged ceiling tiles. Make any necessary repairs and replace the ceiling tile.
  • C/6.4 Observed fly swatters stored on a shelf above plastic silverware and hanging on the dipper well of the front freezer. Store pest control devices separate from food and equipment. Use fly swatters in non-food/equipment areas only in order to prevent possible contamination.
  • C/3.2 Observed a container of waffle powder lacking a label.
  • Label all unidentifiable substances (white, clear, etc) in order to prevent confusion or misuse.
  • 4.4/3.2 Observed pooling water on the bottom shelf of the reach-in cooler on the cook's line. Make any necessary repairs and maintain foods protected from possible contamination. Ambient air temperature of this unit was properly below 41°F today.
  • 4.5 Observed ice build-up on the handle area and the top shelf of the 2-door upright freezer. Make any necessary repairs and maintain foods protected from possible contamination.
  • C/4.1 Observed a dead battery on the digital thermometer. Maintain thermometers accurate and in good working order in order to monitor food temperatures. PIC replaced the battery and the thermometer was accurate to 32°F in an ice bath today.
  • NOTE: Observed a container of coleslaw in the walk-in cooler with an expiration date of 9/3/2011. Check and rotate stock as necessary.
  • -Take steps necessary to correct repeat violations.
  • -Observed hamburgers and coney sauce hot holding properly above 135°F.
  • -Observed potentially hazardous foods cold holding properly throughout.
  • -PIC reported proper thawing procedures, under refrigeration.

  • R/*3.4 The chicken tenders were holding at 130°F. Potentially hazardous foods shall be maintained hot at 135°F and above to limit the growth of organisms.
  • R/*4.5/4.6 The following food contact surface of equipment are soiled:
  • R/- the can opener blade
  • R/- the ice guard inside the ice machine
  • Clean and sanitize food contact surfaces to sight and touch.
  • C/R/*4.4 The quat sanitizing solution in the wiping cloth bucket was at 0ppm. Maintain quat sanitizing solution at 200ppm.
  • *4.1 Two out of five long stem thermometers were accurate in an ice bath. Discard the innacurate thermometers if they can't be calibrated.
  • 4.4 The following areas were damaged/uncleanable:
  • R/- top interior frame/seam on the Frigidaire ultra freeze unit is cracked
  • Repair the unit to provide a cleanable surface.
  • R/3.5 Provide ingredients on the labels for the ice cream cake logs.
  • R/5.3 Provide an indirectly drained food preparation sink for a washing tomatoes and leaf lettuce.
  • 4.8 Boxes of cups are stored on the floor. Store the cups 6 inches off the floor.
  • 4.0/4.4 The screws on the ice guard inside of the ice machine appear to be rusty. Replace these with a screw that is noncorrisive.
  • 4.5/6.4 The following areas were soiled:
  • R/- floor under in the walk-in freezer shelves
  • R/- floor in the walk-in cooler
  • R/- floor below the green dry storage shelves
  • R/- floor below the cake freezers in the back kitchen
  • R/- floor below the walk-in cooler shelves
  • R/- floor below the soft serve ice cream units
  • Clean these areas.
  • R/6.4 The ceiling tiles throughout are water damaged and stained. Replace the ceiling tiles with smooth, durable nonabsorbent tiles.
