--Food, such as onions, located in the cold hold unit, were not being maintained at or below 41 degrees F. The onions were cut at room temperature and then put directly into the cold hold unit. Do not put room temperature food in the cold hold unit. Cool down room temperature food to 41 degrees F in the refrigerator before putting it into the cold hold unit.

--There is a gap under the back door of the facility. Install a door sweep to prevent rodent/pest entry.
- Controlling Pests, Removing Dead or Trapped Birds, Insects, Rodents and Pests (46.981(k)(l))
--Live roaches were observed on glue traps at the hand sink. Continue to take appropriate action to eliminate the roaches, such as: checking incoming deliveries for roach activity, regular cleaning of food debris, setting out glue traps, sealing all gaps within the facility and working with the contracted pest control operator. Continue professional extermination twice monthly until the roaches are eliminated.
