A complaint investigation was conducted due to RFS#16-0034. Complainant stated they had purchased a sandwich from this establishment which had spoiled meat. The establishment offered to replace sandwich. During investigation no temperature violations or date marking violations were observed. Employees stated that had not had any issues with spoiled product and all product that was currently in use was prepared before they came in for their shift. All coolers were holding product at or below 41F. Date marking was observed proper with only one product marked as today being the last day of use. Providing HACCP: Date Marking, Potentially Hazardous Foods, and Refrigeration temperature guide. No further investigation needs to be conducted at this time. During the investigation it was discovered that the ice bin and maker was contaminated with a yellow/green substance. Machine has been embargoed. Owner has contacted a service company to properly clean and sanitize machine. Company will arrive sometime tomorrow morning (5/5/15). Contact Peoria City/County Health Department when machine has been cleaned and sanitized for a recheck and embargo release. 309.679.6161. This is a 30 day routine inspection for the Subway @ Mapleton Mini Mart. There was no license yet printed for them. They did receive a license number as noted on the front page of this inspection report. HACCP: Discussed proper storage of personal items to ensure that foods and equipment are protected from potential contamination. Note: Tuna is made from prechilled ingredients. Meatballs are receievd frozen and portioned out into containers as needed. Continue to maintian proper procedures and operations and routine hand washing. Frozen foods are frozen. This the is the final inspection for the Subway. There was no license yet printed, it was confirmed with the secretary that the license had been paid for. This will be dropped off on Monday. The Subway is sharing the restroom with the Mapleton Mini-Mart. There is no physical barrier (door/gate or otherwise) separation between the Mini Mart and the Subway. It is possible that the Subway may remain open 24 hours a day. Quats is the sanitizer used in the establishment. There is a 4 compartment sink that will be utilized in the following manner: Wash, Rinse, Sanitize, Vegetable Washing. There were no major violations at the time of final inspection. Ensure that all areas are cleaned and sanitized prior to placing foods in coolers or on shelves. Clean all floors routinely. Ensure that routine and proper maintenance of the septic system is maintained. The establishment is approved to operate.
